Our Mission: We guide and support individuals and their caregivers coping with illness, aging, dying, and loss by providing compassionate medical, emotional, spiritual, and personal care. Our Vision: Centrica Care Navigators will be the recognized leader in compassionate care for all people in our communities coping with aging, dying, and loss.
Description
Originally founded as Hospice Care of Southwest Michigan, Centrica Care Navigators is a nonprofit organization that was created to serve the communities where we live. We focus on you as a whole person - not just your illness or disease - and surround you with a circle of care and compassion for your body, mind, and spirit. We’ve established a long legacy of innovating to provide a better, more comprehensive circle of care for you and your family. During our 40-plus years of caring for our community, we’ve continually expanded our services and reach to serve more of our neighbors at different points in their care journey.
We introduced the first inpatient hospice sites in our region. We are pioneers of delivering palliative care at home. Our adult day services give families vital support and freedom. And we’ll keep striving to improve as medical science and technology advance - all the while staying centered on your needs.
Centrica Care Navigators serves Allegan, Barry, Berrien, Branch, Calhoun, Cass, Eaton, Kalamazoo, St. Joseph, and Van Buren counties.
