To provide everyone the opportunity to spend the remaining months of life's final journey in familiar surroundings...alert and free of pain...among the people and things they love. We are dedicated to making this possible.
The Hospice philosophy is based upon caring for the patient and family who are experiencing a terminal illness. Emphasis is placed on improving the quality of life through symptom management, pain control and compassionate support in the comfort of the patient's home or nursing home. The purpose of Hospice care is to enable patients to live as fully and comfortably as possible by ministering to their physical, emotional, social and spiritual needs, as well as to those needs of their loved ones. Hospice always affirms life, and neither hastens nor postpones death.
THE HOSPICE TEAM
Hospice care is not provided by one person alone. Pathways Community Hospice utilizes a team of highly qualified professionals from many disciplines and experienced Hospice volunteers. All team members specialize in easing the physical, emotional and spiritual pain that often accompanies a life-threatening illness.
The members of our Hospice Team consists of:
· Physician of the patient's choice
· Registered nurse
· Social worker
· Spiritual caregiver
· Grief counselor
· Home health aide
· Medical director
A person is eligible for hospice care from Pathways Community Hospice if:
- The patient's physician orders a hospice evaluation/admission.
- The patient lives within the Pathways Community Hospice service area.
- The patient has a primary caregiver or is in a long-term care facility.
- The patient has a life expectancy of 6 months or less.
We are licensed by the stare of Missouri to serve the residents in the following counties:
- St. Louis City and County
- Jefferson County
- St. Charles County
- Portions of Franklin and Lincoln County
