Hospice Companion Volunteer

Our hospice companion volunteers visit with our patients a few times a month. These visits provide companionship to our patients and for patients living at home our volunteers provide much needed relief to primary caregivers to take some time for self-care. Common activities our volunteers do with our patients include Reading to the patient Sharing stories Singing to the patient Sharing stories Taking walks or just getting the patient outside on a nice day Playing games Home patients can sometimes use additional support including light house work, meal prep, or yardwork At Transitions Care we rely on the support from our volunteers to share their time and talents to provide the highest level of care. We believe volunteers are a vital part to the patient care team as they provide comfort and companionship to the patient like no other. We have several volunteer opportunities to choose from that can be tailored to fit your busy schedule.

Providing Companionship for Older Adults: Adopt a Senior Patient

The pandemic has given the general population a deeper understanding of what life is like for older adults who may experience loneliness on a regular basis. And though we all have struggled to make connections during COVID-19, older adults have been overwhelmingly impacted due to strict social distancing measures and limited interactions. Nobody knows this better than caregivers, who may have been their only social contact over the past year. At Transitions Hospice we help provide the highest quality of care to our patients. Volunteers are a vital part of the patient care team. We are looking for families that would be open to adopting a senior for the year. This program would involve getting everyone in your family involved by having anyone 18 or younger write to your senior each week about their lives, questions, etc. Then the adults in the household would then come in once a week to read the letter(s) to the senior and help them communicate back to your child. This creates a family bonding experience for everyone to enjoy. Our patients have lived amazing lives and enjoy sharing their stories with others and having someone for even an hour to talk to makes the world a difference in their lives.
