Heartland Hospice is a "We Honor Veterans" Partner

Heartland Hospice is dedicated to serving our country's Veterans with excellent End-of-Life Care specifically designed to meet their needs. We want to expand our Veteran-to-Veteran Hospice Visitation Program. If you would like to remember, honor, and support America's living heroes, please call today. Many people think hospice is only about end-of-life care, but it’s so much more than that. We help people with life-limiting illnesses, along with their loved ones, make the most of every moment.

Description

We provide caring, compassionate end-of-life care to patients and their caregivers throughout the Lehigh Valley area. Patients reside in Skilled Nursing facilities, Assisted Living facilities, and personal residences. Our holistic care is provided by a professional team which includes our Medical Director, RN's, LPN's, CNA's, Social Workers, Bereavement Coordinator, Spiritual Care Coordinators, Volunteer Coordinator, and Volunteers.
