ProMedica Hospice is dedicated to serving our country's Veterans with excellent End-of-Life Care specifically designed to meet their needs. We want to expand our Veteran to Veteran Hospice Visitation Program. If you would like to remember, honor, and support America's living heroes, please call today. We will achieve this by recruiting professional, caring staff, encouraging patients to become active participants in care, and by creating an atmosphere of supportive leadership. We value strong, mutually beneficial relationships with referral sources, employees, patients and caregivers and community at large.

Description

We provide caring, compassionate end-of-life care to patients and their caregivers throughout the Lehigh Valley area. Patients reside in Skilled Nursing facilities, Assisted Living facilities, and personal residences. Our holistic care is provided by a professional team which includes our Medical Director, RN's, LPN's, CNA's, Social Workers, Bereavement Coordinator, Spiritual Care Coordinators, Volunteer Coordinator, and Volunteers.
