Role Overview: As an Information & Referral Support Line Worker for the Healthy Aging Response Team (HART), a department of Daly City Partnership, you will be an integral part of our team. This role is pivotal in ensuring the well-being of individuals within the community served by HART. This position responds to incoming and provides info. and referrals for critical connections to social and health services to older and disabled adults. Follow-up and check-in calls are provided to ensure client connectedness and support.

Requirements:

  • Excellent interpersonal and communication skills; ability to listen deeply, be patient and compassionate and practice good judgment.
  • Desire to be of service to older adults and adults with disabilities; ability to provide respectful and compassionate support.
  • Detailed and careful recordkeeping for data entry and important client case notes.

Preferred Skills:

  • Basic computer skills, simple data entry (training provided)

Training: Participate in a 30-40-hour comprehensive training that provides a working understanding of community-based services and personal communication methods that promote compassionate, caring phone engagement. Continuing education is provided.

Time Commitment: 4 hours per week, choice of shifts from 8:00 a.m. to 5:00 p.m. Participation in training, occasional in-person sessions, and completion of necessary onboarding processes.

Our Second Home Early Childhood Family Resource Center is Daly City’s first community center dedicated to the needs of young children and to providing support for the adults in their lives.

Healthy Aging Response Team (H.A.R.T.)

HART is a collaborative effort of local organizations that have come together to serve the rapidly changing needs of adults 50+ and persons with disabilities. It promotes a sense of community-building by allowing peers to volunteer and help their neighbors.

Daly City Community Resource Center (DCCRC)

The DCCRC provides support and quality crisis intervention services, either directly or by referral, empowering individuals/families towards self-sufficiency.
