GENERAL RESPONSIBILITY:

Tuck-In volunteers are available to contact patients/families either by phone or in person for the purpose of obtaining specific information that will assist in preparing patients for evenings or weekends. In addition, these volunteers provide emotional support as appropriate.

ESSENTIAL FUNCTIONS:

  1. Review request and assignment with referring case manager and/or volunteer manager.
  2. Make telephone contact or visit patient location: home, inpatient facility, nursing home, group home, assisted living, etc.
  3. Conduct assessment of needs.
  4. Provide emotional support to patients/family as appropriate.
  5. Provide case manager and/or volunteer coordinator with the information obtained.
  6. Follow all policies and procedures for tuck-in services.
  7. Understand limitations of the role of the volunteer in this capacity and refer to appropriate team members as necessary.
  8. Initiate communication with appropriate staff when concerns arise.
  9. Document all interventions according to policy.
  10. Honor time commitment.

REQUIREMENTS:

  1. A mature individual, supportive of the hospice concept, comfortable with his/her own spirituality and willing to serve others in a volunteer capacity.
  2. Successful completion of the hospices volunteer training and orientation program.
  3. Previous volunteer experience may be helpful.
  4. Ability to work within the interdisciplinary group.
  5. If making home visits, must be a licensed driver with an automobile that is insured in accordance with state/ or organization requirements and is in good working order.
