GENERAL RESPONSIBILITY: Tuck-In volunteers are available to contact patients/families either by phone or in person for the purpose of obtaining specific information that will assist in preparing patients for evenings or weekends. In addition, these volunteers provide emotional support as appropriate. ESSENTIAL FUNCTIONS: Review request and assignment with referring case manager and/or volunteer manager. Make telephone contact or visit patient location: home, inpatient facility, nursing home, group home, assisted living, etc. Conduct assessment of needs. Provide emotional support to patients/family as appropriate. Provide case manager and/or volunteer coordinator with the information obtained. Follow all policies and procedures for tuck-in services. Understand limitations of the role of the volunteer in this capacity and refer to appropriate team members as necessary. Initiate communication with appropriate staff when concerns arise. Document all interventions according to policy. Honor time commitment. REQUIREMENTS: A mature individual, supportive of the hospice concept, comfortable with his/her own spirituality and willing to serve others in a volunteer capacity. Successful completion of the hospices volunteer training and orientation program. Previous volunteer experience may be helpful. Ability to work within the interdisciplinary group. If making home visits, must be a licensed driver with an automobile that is insured in accordance with state/ or organization requirements and is in good working order.

Affinity Care Hospice’s mission is to provide the terminally ill patient and their loved ones with the support and care they require to live their lives comfortably. Our Hospice team strives to enable the patient and family to maintain dignity and quality of life. Our goal is to provide physical, emotional, and spiritual comfort. We strive to empower the patient and family by providing ongoing support for family and friends for as long as it is needed. We believe that Hospice care should be available to all persons with an illness for which there is no cure or for persons who elect not to attempt a cure, resulting in a limited life expectancy. Title/position: General Volunteer Duties/Expectations: Our team of Social Workers, Nurses, Aides, Counselors, and Volunteers come alongside and help with cares and concerns the family has during this difficult time. As a volunteer you have the opportunity to get to know the families and patients on a personal level. We have several opportunities to help with patient directly or indirectly: Provide Companionship to the patient and/or caregiver one to two times a month. At this time this can be through phone calls or in person visits for our facility patients and in person visits for our home patients. Pick up groceries/supplies for patients and families and deliver them Deliver blankets to patients in facilities and at home through our Affinity Care Blanket program Time commitment: 5 hours a month Qualifications and requirements: Familiarity with end-of-life care is a plus but not a must. Orientation is approximately 2 hours and is all online. You need to have a passion for helping others who just need someone to listen to them or be there for them. Must preserve confidentiality of client identity and information. A Background Check and TB Screening are required, which are all paid for by Affinity Care Hospice & Palliative Care. Also must have a valid drivers license and car insurance. Benefits: You can make a difference in the lives of patients who are going through the final stages of their life. You will also be a comfort to the family knowing their loved one has someone visiting with them and making sure they are not alone.
