Purpose:
CalAIM is an innovative program designed to address local community residents that often have negative health outcomes. Highly trained staff work with Medi-Cal clients to understand and maximize their healthcare benefits through care coordination and case management for housing support. Staff have long-term service relationships with their clients, addressing their health-related needs by taking a whole person and individualized approach. This can include working with clients on finding local services that will aid with social, financial, medical, behavioral health, housing, education, and employment needs. Staff follow the client through the web of physical health and behavioral health systems with the goal of increasing physical and behavioral health outcomes while increasing social connectedness.
Worksite: Lifeline’s Vista Campus
Key Responsibilities:
- Provide general handling of intake line, responding to incoming calls and voicemails
- Screen clients for programs, scheduling appointments with correct staff members as necessary
- Achieve high level of familiarity with program, including services and corresponding staff
- Provide general clerical support, including filing and data entry, as needed
- Additional responsibilities as assigned
- Contact potential counseling clients and schedule intake appointment
- Conduct in-person, one-on-one assessment interviews
- Gather specific information regarding client’s history, background, and needs.
- Complete detailed paperwork
- Perform data entry
- Create new client files
Desired Attributes:
- Bilingual (Spanish) preferred
- Office experience preferred
- Patient, helpful, friendly demeanor on the phone and in person
- Ability to empathize with clients while adhering to program guidelines
- Excellent written and verbal communication
- Ability to multi-task and prioritize
- Proficiency in Microsoft Office applications ( Word, Outlook, Excel) required
Time Commitment:
- Minimum commitment of 1 6-hour shift per week (or 2 3-hour shifts)
- Available days/shifts: Monday - Friday during business hours
