Medical Advocate Program

Help give a senior or disabled person emotional strength to manage their health care appointments. Be there to assuage concerns about seeing the doctor and help keep track of their medical issues. You will be matched one-to-one with a participant in our community based CONNECT program for people with behavioral health hurdles to improve their access to care.

State Funded Home Care Agency whose programs and services are targeted to low-income residents of Somerville and Cambirdge, age 60 and over, who need assistance managing their daily activities.
