Growing non-profit in need of Case Managers.
Hospice Pet Advocates is a non-profit organization founded by health care providers to support terminally ill patients in pre-planning and providing care to their beloved pets. We offer a range of services designed to support clients pets during all phases of illness, as well as after care and emergency direct rescue.
We are in great need of experienced health care providers to donate case management services for just a few hours a month. Case Management for Hospice Pet Advocates is short term and generally includes identifying specific needs to delegate to the team, and providing emotional support during difficult transitions.
Background experience providing in-home health care services is a must. Experience working with animals is a plus.

Patient Pet Advocates provides pre-planning assistance, education, emergency services and advocacy for pet owners facing a terminal or life-altering medical issue.
Description
PPA was co-founded by a medical social worker and a hospice nurse when they saw a need first-hand.
PPA works with patients, their families and health care professionals to provide pet-related services to those facing end of life, transitions in care or life-altering medical issue.
- Raising awareness about the importance and necessity of pre-planning for pets.
- Educational programming for patients and care providers about pet issues.
- Pre-planning counseling, networking and direct assistance
- Emergency rescue services
- Placement with a partner adoption agency.
