Health Innovators convenes the medical community and articulates a collaborative vision for improving the quality, safety, and efficiency of healthcare through an integrated population management strategy that enhances access to providers, improves caregiver communication and fosters a team approach to care delivery. The patient-centric team approach is supported through in-office and community-based clinical services, and integrative health information technology (HIT).

Integrative HIT facilitates connectivity across the care site continuum and enables the development of evidence-based clinical decision support tools, and performance measurement systems. They also support patient engagement providing information to empower them to be active participants in fostering and maintaining their own health.

In addition, HI engages payers in designing value-based insurance products that promote evidence-based workplace wellness, primary prevention and disease management to keep our community healthy and reduce the clinical and financial burden of disease.

Purpose of Position:
To meet the growing demand for providing health promotion, disease management, and health enhancement services to the diverse populations that HI serves.

Responsibilities:
The Health coach will be part of a multi-disciplinary team responsible for providing care coordination services to HI's growing book of business. The Health Coach will engage plan members, patients, and community members in identifying lifestyle modification opportunities, establishing goals and action plans, and conducting follow-up to support healthy, sustainable behavior change. Additionally, this position will support a variety of disease management and wellness services including, but not limited to:

  • Providing health coaching activities across the continuum of care in order to facilitate and promote high quality, cost-effective outcomes, focused on the whole patient orientation and self management decision support and aims to minimize any fragmentation of health care delivery.
  • Acting as the liaison between the patient/member, the patient/member's support network, treating physician(s), and ancillary providers utilizing a multi-disciplinary approach to assess, identify, plan, health goal setting and measure optimal clinical outcomes.
  • Employing behavior change/motivational interviewing skills to assess readiness, health goal setting short and/or long term needs, engage member's plans for change following standard policy and procedures, clinical guidelines and national evidenced-based criteria.
  • Working collaboratively with all HI team members and stakeholders including but not limited to; physicians and their offices, hospital staff, corporate clients and local employers, health agencies, partner hospitals, etc. to identify individuals who require disease management, wellness, and prevention services.
  • Mastering the use of HI e-tools to identify and target needs of risk populations.
  • Supporting the development, implementation and evaluation of CHI programs and outreach activities to improve optimal health outcomes. May include onsite program promotion, biometric screenings, managing worksite wellness challenges, health education content development, and providing health education lectures, targeted mail campaigns to address identified gaps, etc.
  • Acting as patient advocate responding to and working to resolve patient concerns or barriers.
  • Performing additional duties as assigned.

Position Requirements:

Candidates must have:

  • A Bachelor's degree in health related field (i.e. health promotion, public health, nutrition, kinesiology, etc.) or LVN licensed in California and High School diploma or equivalent
  • At least two years experience as a health coach or providing impactful health promotion/disease management services(clinical setting preferred)
  • Knowledge of managed care concepts, health promotion/disease management strategies, and trans theoretical model
  • Knowledge of general healthcare practices, office workflows and medical terminology
  • A strong commitment to team environment dynamics with the ability to contribute expertise and follow leadership directives
  • Ability to thrive and be dynamic in a constantly evolving environment
  • Ability to develop health education materials and tailor information at the appropriate level of literacy
  • Strong verbal and written communication skills and ability to apply motivational interviewing techniques
  • Strong organizational and prioritization skills
  • Strong proficiency in Windows-based database applications
  • Ability to work independently and prioritize a heavy workload with minimal supervision
  • Ability to work with patients/members to develop health enhancement strategies to improve health outcomes and self-management
  • Exceptional problem solving skills and adept at conflict resolution
  • Strong customer service skills and ability to function among a high performing team
  • Ability to communicate in a timely manner with all stakeholders
  • Ability to effectively establish rapport with patients/members in person and via telephone
  • Ability to develop and implement wellness challenges and engagement activities

Preferences:

  • Experience with providing patient education and chronic condition management
  • Managed care experience
  • Health Coach certification
