About

Diabetes PATH (Personal Action Towards Health) is a FREE 6-week, 2 ½ hour workshop that helps people with diabetes, pre-diabetes or people living with a person with diabetes learn skills to help better manage diabetes and related health conditions.

What can you do to help others and yourself?

Become a DIABETES PATH Leader!!
We are looking for people who are motivated to help themselves and to assist others to manage diabetes and related health conditions.


Skills: People skills, Able to read/write/speak English

Good match for: Anyone, People 55+ group

Requirements and commitment: At least 18 years old
Able to pass a criminal background check
Training
After training, facilitate one 6-week workshop in the following 6 months

Training: Monday & Tuesday, September 26 & 27
Monday & Tuesday, October 3 & 4
All four days meet from 9:00 am-5:00 pm
