Transitions is a free program to offer additional companionship, caregiver relief, or other support to community members with a life limiting disease and their family in their home or nursing home. The individuals served are not yet elligible for hospice service or choose not to use hospice, but who need additional support to age in place. The program is offered by BJC Hospice and is run by a staff social worker but is otherwise 100% dependant on volunteers. This volunteer role offers a possibility of long-term relationships with those served as the volunteer is able to continue to work with should they later become a BJC Hospice patient.
