Mission Statement
Connections to Care exists to serve seniors, living within the northwest suburban Cook County area, who require assisted transportation services to attend medical appointments, which allows them to age in place in their community with dignity. This program provides a vital lifeline for hundreds of local seniors so that they may get to their life-sustaining medical and dental appointments.
Description
Interested in being one of our amazing drivers? It's easy, fun and incredibly flexible -- we work with you to accommodate your schedule. You can drive for us as much or as little as you'd like, and we set up our rides at least a week in advance, so there's no last minute calls. You provide your own vehicle; we provide additional 'umbrella' insurance coverage.
Seniors need assisted transportation for everything from dialysis and blood tests to pacemaker checks, physical therapy and wound care. We territory is Chicago's NW suburbs with our boundaries being: Lake Cook Rd (N), Barrington Rd (W), Biesterfield Rd (S) and Greenwood Ave (E). Hospitals included in this area include Northwest Community, Lutheran General, Glenbrook, Alexian Brothers and St. Alexius. You may choose where you'd like to drive.
This tremendously successful program continues to grow, particularly as our aging population lives longer. We're here to help seniors stay independent for as long as they're able.
