Mission Statement
Mission: To promote the health of medically under-served children in the greater Roanoke Valley by ensuring comprehensive healthcare, strengthening families, and coordinating resources in a public-private partnership.
Target Population: Low income pregnant women and children birth to kindergarten entry.
Description
Program: As an early intervention program, CHIP helps low-income children maintain access to comprehensive healthcare and public health services, contributes to measuring and monitoring healthy child development, and serves as a coordinated referral service to each enrolled child.
Services: Monthly Home Visits with a Community Health Nurse Case Manager; Health Care Coordination; Kindergarten Preparation; Asthma Case Management; Semi-annual Dental Varnish for children 6-36 month; Family Strengthening Services including Employment and Education Support; Mental Health Case Management; Prenatal Case Management; Transportation to Medical Appointments and Developmental Evaluations for Enrolled Children; Community Referrals; Books, Home Safety Items, Developmental Toys
