Description

Tandem 365 is an innovative program to help participants navigate the complex healthcare system. Through the collaborative efforts of Clark, Holland Home, LifeEMS, Porter Hills and Sunset, seniors are assisted with complex medical conditions to safely stay in their homes as long as possible. Each participant has access to comprehensive community and health services through a team of navigators. Tandem needs volunteers to donate time, energy and experience in a variety of areas including yaradwork, handyman services, friendly home visits, phone calls, transportation and more. Volunteers are a vital part of our success!
