• Provena Hospice is expanding to the Frankfort area. We are a professional Medicare based hospice that provides compassionate care and services to all patients who live in our area and who meet the criteria defined by Federal regulations or by the State of Illinois licensure laws. We utilize an interdisciplinary team approach, providing caring support to our patients who have a life-limiting illness and have been diagnosed as having six months or less to live. Our focus for our patients is on their quality of life. We try to offer each person comfort care and spiritual/emotional support for the body, mind and spirit. The team includes the hospice nurse, nursing assistant, social worker, Chaplain, bereavement counselor, the medical director, family members and volunteers. We serve our patients and their families in their homes and/or nursing homes, providing respite care, bereavement support and inpatient care through our affiliated hospitals.
