The mission of Colorado Health Network (CHN) is to equitably meet the evolving needs of people affected by HIV and other disease through prevention, care and advocacy.
For more than 25 years, Southern Colorado Health Network (formerly SCAP) has been providing comprehensive and individualized care to men, women and children in southern Colorado who are living with HIV/AIDS.
As the only AIDS service organization in Southern Colorado, SCHN’s geographic service area encompasses 25 counties in Southeastern Colorado spanning from the central mountains of the state all the way south to the Colorado/New Mexico border, and as far east as the Colorado/Kansas border.
Today, the organization’s programs include case management, access to healthcare, mental health and substance abuse counseling referrals, a food bank, and legal referrals, in addition to a wide array of prevention outreach, testing, and education programs.
