Description

Person Centered Partnerships is a non profit, human service agency that provides comprehensive support services to adults who have disabilities and their families. The mission of Person Centered Partnerships Program is to empower people who have disabilities to make meaningful life choices. The agency accomplishes this mission by offering a variety of support services that provide opportunities to live, work, volunteer and socialize in the community. Person Centered Partnerships is currently developing and enhancing support services to meet the changing needs of the people who are served by this agency. Beyond a Promise Day Supports Provide was created to serve as a continuum of care for people with disabilities and to reduce service fragmentation. Beyond A Promise is comprised of a Day Supports program that includes employment and training programs, leisure and recreational skills, computer, education and literacy. Day Supports provides activities that focus on life skills, community integration, social inclusion, leisure and recreation as well as opportunities for paid and volunteer work. Participation in the community at large is an integral role of this support. A variety of trainings, therapies, and group discussions are offered in order to meet the needs of each individual. Trainings take place in both group and individual settings to best support and improve the quality of each person's life. Daily discussion groups address a wide variety of topics such as human rights, fire safety, health and nutrition, and current events. Music therapy teaches individuals about rhythm, harmony and song development.

Facilitating Community Integration and social inclusion is another essential role of the program. We strive to organize numerous recreational and social events including classes, dances and special trips. In addition to regularly scheduled trips to theaters and restaurants, we will offer off -site cooking classes, a gardening group, and a walking group. Participants will work hard each year to produce wreaths, quilts, blankets and other arts and crafts for those in need. The Community Integration specialists also work to identify opportunities for individuals to pursue their interests through civic, social, religious, sporting, and hobby oriented clubs and organizations within the community at large
