Description
Celtic Hospice & Palliative Care Services, serving Allegheny, Armstrong, Beaver, Butler and Westmoreland counties, is dedicated to providing relief and comfort to individuals through symptom management and pain control. Celtic Hospice offers an interdisciplinary team approach to care for those individuals with progressive, chronic or debilitating illnesses, and those facing end of life situations. The program is designed for patients who require comfort measures to enhance their quality of life and is directed at treating the person rather than the disease. Celtic Hospice's specialized clinical services provide:
24-hour on-call nurses who makes home visits or send the appropriate team member if needed between scheduled visits
Non-invasive treatment of symptoms and pain control
Enhancement of functional status and teaching to caregivers
Emotional support for relief of worry, anxiety and depression
Key goals of Celtic's Hospice & Palliative Care Services program include:
Accurately assessing the patient's physical, emotional and psychological status to develop a care plan
Advocating for the client/family
Providing emotional support with decisions and ongoing care options
Assessing the client/family's acceptance/understanding of the potential of hospice and palliative care
Educating the client and family on the disease process and the differences between the types of care
Developing trust -- providing a service in a manner that is as seamless as possible and provides for future support
Enabling the patient to maximize their dignity and quality of life.
