Description

Northwestern Memorial Hospital Palliative Care and Home Hospice Program strives to provide the best patient and family experience by enhancing the dignity and quality of life of those facing life-limiting illnesses. We aim to achieve this by addressing the physical, emotional, and spiritual suffering of patients and families through an interdisciplinary team of nurses, social workers, chaplains, home health aides, massage and art therapists, and volunteers. Our program services the entire Cook County area.
