At Southern Care our mission is to improve the quality of life for those people and families who may face a life threatening illness, by providing with unique, specialized care and meeting each familes' needs one at a time.

Description

We have a team of professionals along with our volunteers which include a clinical director, medical director, RN casemanager, Hospice Aide, Social Workers, and Chaplain which visits our patients at home or in an assisted living home.
