Quality Health Care and Therapy in a subacute setting for people recovering from illness and falls, people with disabilities, and people with dementia. The goal is to improve or maintain the patients fuctional level through therapeutic modalities for transition home in 2-3 weeks.

Description

A short-term 24 bed subacute/skilled SNF providing Physical, Occupational, Speech, and Recreational Therapies to patients discharged from an acute setting but not ready to return to independence at home. Activities are provided throughout the day.
