Stratis Health

Description

Stratis Health is an independent nonprofit organization that has led collaboration and innovation in health care quality and patient safety for over 40 years. We work across settings of care to improve the health of individuals and communities, and to improve the processes used to deliver care.

Stratis Health facilitates learning and action networks across communities, and provides direct technical assistance to health care providers. We have four key program areas: health equity, health IT, rural health and implementing the goals of the Medicare Quality Improvement Program in Minnesota.

We have supported the Medicare Quality Improvement Program since its inception in 1971. We lead Lake Superior Quality Innovation Network serving Minnesota, Michigan and Wisconsin, as one of 14 Medicare Quality Innovation Networks across the country working to implement national health care quality priorities on the local level. This work includes improving transitions between settings of care, reducing health care acquired infections and increasing rates of preventive services like immunizations.
