To engage the community in awareness and understanding of Advance Care Planning through educational programs and individual guidance in the completion of one's Advance Directive.

Description

The Advance Care Planning Coalition of Eastern Virginia represents a unique collaboration of four leading health systems and four area agencies on aging. These are Bons Secours Hampton Roads, Chesapeake Regional Mediacal Center, Riverside Health System, Sentara Healthcare and the Peninsula Agency on Aging, Senior Services of Southeastern Virginia. Bay Aging, and Eastern Shore Area Agency on Aging. The founding members, along with public and private partners of health and human services, promote community engagement through the As You WishAdvance Care Planning program.

The Coalition currently operates in 16 counties and 9 cities in eastern Virginia. Information and guidance to anyone in the Commonwealth can be provided by phone or electronically.
