Mission Statement

Heartland Home Health Care and Hospice is a growing company dedicated to providing compassionate high quality home health care and hospice. We will achieve this by recruiting professional, caring staff, encouraging our clients to become active participants in their care, and by creating an atmosphere of supportive leadership. We value strong, mutually beneficial relationships with our referral sources, employees, clients and their families and the community at large.

Description

Heartland Hospice serves those facing terminal illnesses in their homes or in a skilled nursing facility. It is our goal to provide palliative and supportive services to meet the physical, psychological, social and spiritual needs to those facing the end of their lives.

Care is provided through our interdiscip;inary team of physicians, nurses, social workers, spiritual care coordinator, volunteers and bereavement support.

Hospice Volunteers assist individuals to live with dignity and comfort at the end of their lives.
