Mission Statement

Assyrian Family Health Alliance, NFP mission is to create a community-based diabetes self management education program that will serve as an effective intervention for glycemic control among adults and young adults in the Assyrian American community in Skokie, Illinois and other local communities where the Assyrian American community resides. The goals of the DSME program are to improve metabolic control and quality of life, to reduce diabetes-related complications, and to minimize healthcare costs.

Description

The community based program will be an interactive, collaborative process that can equip the young adults as well as the adults with basic knowledge to manage their diabetes while focusing on their self identified problems and goals.
