Mission Statement
The mission of Trinity Hospice is to provide quality hospice care to patients with life limiting illnesses and to their caregivers and families. The goal of hospice is to add quality of life when quantity is limted. Trinity Hospice recognized dying as a part of the nornal process of living and focuses on maintaining the quality of remaining life. Trinity Hospice offers palliative care to all terminally ill patients and support to those patients and families without regard to diagnosis, gender, sexual orientation, national origin, race, credd, disability, age place of residence of ability to pay
Trinity Hospice values and honors the following twelve concepts of hospice care. Informed consent, Patient/Family as a unity of care, Patient/Family rights, Dying as a natural process as living, palliative care, Interdisciplinary care, Volunteer, Services always available. Admission based on need, Bereavement care, Continuous learning form patients/families and ongoing research.
Description
When a patient and family become part of the Trinity Hospice program, they are able to help and assist our patients and or family in many ways.
Patient Care Volunteers: Visit with the patient and or family members, Provide companionship, play cards, dominoes, etc. Provide listening ear. write letters or notes, ready to patients.Share stories. Polish nails. Acitivites to enhance the lives of our patients.
Special Project Volunteers: Work with public relations coordinators: represent a service organization or groups who are already organized to provide a unique type of volunteer activity. Help with socials at nursing homes, administrative office help in our office. Making copies, filing etc. Making cookies and special treats and delivering them to our patients. Helping with vintage hat shows we do for nursing homes.
