Our mission is to empower individuals with mental illness to function more independently in their social, living, and working environments, and to secure decent, safe, and affordable housing for them in Shawnee County.

Description

Breakthrough Living Program's psychosocial rehabilitation promotes recovery, full community integration and improved quality of life for persons who have been diagnosed with a mental health condition that seriously impairs functioning. Breakthrough Living Program is collaborative, person directed, and individualized. Our staff focuses on helping individuals re-discover skills and access resources needed to increase their capacity to be successful and satisfied in their living, working, learning and social environments of their choice.

Care is provided in an intimate environment without professional authoritative shields and barriers. The assessment of needs and care is differential based on the unique needs and environment of each person. Together, we work with case managers, doctors, therapists, family, and all members of the person's support team on continuing individualized goals and plans and for early intervention. Attempts will be made to modify the environment in terms of rights, responsibilities, services, and social change. Our model of care is social rather than medical and a person's strengths rather than pathologies are empathized. We assist people achieve independence and a high quality of life.
