To enable self-sufficiency by removing health issues as barriers by providing free healthcare services and health education to homeless women and children in Mecklenburg County, NC.

Description

Shelter Health Services (SHS) is an independent free clinic open M-F, 11:30am-6:30pm. SHS currently serves a single site; the Salvation Army Center of Hope, shelter for women and children near downtown Charlotte. SHS provides medical care and nursing care to 50-100 women and children each week. We provide gap care and case management, including physical exams, medication assistance, referrals, health teaching and transportation assistance. Special programs include the Chronic Disease Management Program, TB testing and HIV testing and counseling. We provide care based on our "PRISM" model: presence, respect, information, services and movement.
