The Health Sense Hospice mission is to participate as an active part of the community, in providing and continuously improving the hospice care needs of the patient by delivering value driven, high quality compassionate care.

Description

Our hospice program is a coordinated program of palliative care (relief from pain and suffering) and supportive services provided in both home and inpatient settings. We provide for the physical, psychological, and spiritual care of the dying person and their family. Services are provided by a medically directed interdisciplinary team of professionals and volunteers. We also provide bereavement care to the family following the death of the person.
