Our Mission "To operationalize our vision;The Transition House INC. will create a consumer driven seamless system and clinical infrastructure to support substance abuse ,HIV/CDIS,and Mental Health services for Central Florida most disefranchised residents.

Our vision is "To eradicate public health maladies within our communities by way of community empowering model that is culturally and linguistically proficiant"

We believe in the celebration of life. Our goal is to assist Clients in acomplishing their celebration of life,through hard work ans determination we believe they will be able to mainstream back in society with renewed sel assuredness,essential skills and self-sufficiency to lead productive lives.

Description

The Transition House has been providing transitional Housing ,substance Abuse treatment ,mental Health tx ,employment counseling and job readiness training to the homeless and clients since 1993. The agency performance in meeting the needs of the area's homeless population has been exceptional ;the agency has returned 800 homeless individuals to self sufficiency in the community in the last 3 years.
