Mission Statement

It’s all about helping people.

We provide exceptional care and unparalleled service to patients and families who have placed their trust in us.

We will improve the quality of life in the United States by transforming the delivery of healthcare services

What is hospice?

Hospice provides comfort and support to patients and families facing the final stages of a terminal illness. The goal of our program is to help your loved one live comfortably, with dignity, when a cure is no longer possible. Our team of specially trained professionals delivers expert medical care, pain management and emotional and spiritual support tailored to the needs of your loved one. Counseling and bereavement support services are available to families before and after a patient’s death.

Does hospice mean we are giving up?

Hospice is about making every day count. When a patient is admitted to hospice, it usually means there is no further curative treatment to offer. As clinicians and caregivers to hospice patients, our focus shifts to improving quality of life. Medical treatment continues to address all of the patient’s symptoms, with a special emphasis on controlling your loved one’s pain and discomfort.

What should I tell my loved one?

Our hospice professionals are here to answer your questions about the dying process. We encourage families to be honest with patients about a terminal prognosis. Though it may be difficult, this is the time for you and your loved one to discuss important family issues that should be resolved. Don’t be afraid to be honest with your loved one and say the things that need to be said.

When is the appropriate time to call hospice?

Hospice care is designed to maximize quality, relationships and experiences at the end of a patient’s life. When a patient is told there is no further curative treatment, a referral to hospice should be made.

We deliver high-quality care and so much more.

At LHC Group, we’re here for you - day or night. We are dedicated to providing exceptional hospice care that supports the choices made by you and your family and empowers you to live with dignity and peace during the final stages of life. As a leader in post-acute care, we partner with the nation’s leading hospitals and health systems to bring a seamless, patient-centered continuum of care to the communities we serve.

Hospice patients are admitted within 24 hours of referral and are treated by a specially trained team of professionals. Our commitment to compassionate care and service excellence influences every facet of our hospice program. As a result, our family satisfaction rates exceed 90 percent.

Comfort. Compassion. Dignity. Respect. This is hospice care at home.

Hospice care team

Our interdisciplinary approach to hospice care includes a team of medical directors, nurses, social workers, counselors and trained volunteers. We work with patients and families to create a personalized plan of care, helping to ensure everyone receives the support and information needed.

Trained hospice volunteer program

Our dedicated volunteers are an important part of our hospice team, offering assistance and support to patients and families. Volunteers help with everything from preparing meals and running errands to sitting with patients and writing letters to loved ones. Many hospice volunteers say their experience is life changing.

We Honor Veterans

We understand veterans often carry experiences from their military service that present unique challenges at the end of life. That’s why we’ve partnered with the Department of Veterans Affairs and the National Hospice and Palliative Care Organization to offer a pioneering hospice program focused exclusively on the needs of America’s veterans and their families.

Bereavement support

Our bereavement support services can help you and your loved ones recognize feelings of grief and loss and put them in perspective. The experience of sharing with others who are in a similar situation can be comforting and reassuring.

Online care community

A strong support system is crucial to patients facing a life-limiting illness. Now, patients in our care can easily keep in touch with family and friends online through a private care community. We’re proud to offer free access to the CareFlash service. It takes just a few simple steps to create a secure, invitation-only website that allows patients to involve loved ones in their care.
