Mission Statement

It is the mission of LifeChoice Hospice to provide individualized and the highest quality care to patients who face a life-limiting illness. We extend our care, support and services to our patients,families and the communities in which they reside.

Description

LifeChoice Hospice utilizes a team of individuals to meet the medical, psych-social and spiritual needs of our patients and their families. The team is made up of nurses, nursing assistants, social workers,chaplains and volunteers. We provide care in a variety of settings such as skilled nursing facilities, assisted living facilities and patient's homes. Our goal is to provide comfort and dignity and to enhance the quality of life for those we serve.
