Description

How will you help our elders? · Provide One-to-one Services : Home visitation, meal delivery/preparation, transportation, linkages to community resources, and other support. · Implement Special Projects/Classes : Health, physical fitness, fall prevention, mind fitness, crafts, music, social and cultural activities, and skill development. · Provide Education and Outreach : Provide information to individuals, care-givers, service providers, employers and other stakeholders. · Conduct Capacity Building Activities : Recruitment/management of volunteers; policy, or fund development; marketing services, creation of new programs and similar activities. · Conduct Community Mobilization/Advocacy : Marshall resources to address community issues and help to advocate for funding.

I was inaugurated into the WECARE program, which is a branch of Americorps,on January 22, 2010 and am volunteering at St. Jude Medical Community Services with Senior Services as a Care Navigator. As a Care Navigator, we help Seniors who have recently been discharged with linkages to resources, like transportation to Dr.'s appointments, help with in home safety issues, nutrition, exercise classes, and other resources to help prevent further hospital readmits. It is very rewarding and educational because we all are going down the same road of life and helping Seniors who appreciate the guidance is very rewarding and as we teach and explain about resources we are also learning what we, as Baby Boomers, need to know as we reach our senior years.
