Healthy Choices New York is a statewide initiative that seeks to empower older adults by providing evidence-based health promotion programs in community-based settings. This initiative is funded through the federal Administration on Aging, the New York State Office for the Aging and the New York State Department of Health. The three regions participating in the project include: Broome County, the Capital Region (Albany, Rensselaer, Saratoga, and Schenectady Counties), and New York City. Community partners include (but not limited to) Area Agencies on Aging (AAAs), local Departments of Health, Naturally Occurring Retirement Centers, community service providers, insurers, and faith based communities.
Healthy Choices New York program offerings include Living Healthy & Active Choices.
Living Healthy is a six-week workshop series that empowers older adults and their caregivers to take charge of chronic conditions, such as arthritis, heart disease, osteoporosis, or diabetes by discovering ways to better manage ongoing needs. Living Healthy was developed and evaluated by the Patient Education Research Center at Stanford University. Living Healthy workshops are taught in both Spanish & English.
Active Choices is an evidence-based program developed by the Stanford Prevention Research Center that has been proven to be effective in helping older adults meet their physical activity needs. Each Active Choices participant is assigned a trained activity coach who helps him/her build an individualized exercise plan that is convenient, flexible, and appealing. Regular monthly phone support enables the Active Choices coach to monitor progress, modify exercise strategies as needed, and provide exercise tips.
