Description
Harbor Hospice provides terminally ill residents and their families the care, support and training they desire by offering the following services: Pain and symptom management, emotional and psychosocial support, personal care such as bathing and dressing, spiritual care, volunteer help, basic medical equipment, all medications and supplies. The staff (physician, nurse, pharmacist, social worker, home health aide, chaplain) work with the family to develop a personal plan of care, visit as needed, meet as a team of professionals, assess treatment, and help patients and families attain their end of life goals. These services are provided for the service area encompassing Musekgon, Oceana and Northern Ottawa Counties. Bereavement services are offered not just to hospice families but to community individuals as well as through one to one counseling, specialized groups and mailings.
