Mission Statement
To provide expert care with compassion, comfort, and support for people facing serious illness or nearing the end of life. To meet the physical, emotional, and spiritual needs of the patient and his/her family during the illness and bereavement period by providing comprehensive interdisciplinary care. To provide care consistent with patient / family needs and goals, with patient needs and goals as a priority. To educate individuals and communities in our service area of Muskegon, Oceana, Mason, Newaygo and Ottawa counties and surrounding areas of the needs of patients and their families and to provide training for health care professionals and others which enable them to better meet the special needs of those faced with serious illness.
Description
Harbor Hospice provides terminally ill residents and their families the care, support and training they desire by offering the following services: Pain and symptom management, emotional and psychosocial support, personal care such as bathing and dressing, spiritual care, volunteer help, basic medical equipment, all medications and supplies. The staff (physician, nurse, pharmacist, social worker, home health aide, chaplain) work with the family to develop a personal plan of care, visit as needed, meet as a team of professionals, assess treatment, and help patients and families attain their end of life goals. These services are provided for the service area encompassing Muskegon, Oceana and Northern Ottawa Counties. Bereavement services are offered not just to hospice families but to community individuals as well as through one-to-one counseling, specialized groups and mailings.
