Avalon Gardens Adult Day Health Care

Description

Medical model adult day program provides meals, therapeutic recreation, daily living skills assistance, personal health care, social supports, nursing services, podiatry, vision and dental services, a beautician, and medication management for elders ages 60+ (and for ages 18+ if disabled). The program operates Monday thru Saturday, 9:30 am until 2:30 pm and is certified by the New York State Department of Health.

It is designed for elderly and younger adults who:

-have cognitive, physical or psychological conditions requiring supervision including: Alzheimer's disease or other related Dementias, Coronary Artery Disease, Diabetes, Parkinson's Disease, Stroke, Osteoarthritis, Multiple Sclerosis, Muscular Dystrophy, head trauma, etc.

-require health and/or rehabilitative services;

-require a structured program of emotional and social support and stimulation.
