The mission of Crossroads Hospice is to provide highly competent, comprehensive, and compassionate hospice services to persons experiencing a life-limiting illness and to their families. The hospice is committed to keeping patients in their home environment, whether that is in their own private residence or in a nursing home.
Crossroads Hospice is a community based organization, licensed by the state and certified by both Medicare and the Community Health Accreditation Program (CHAP). From our inception, we have had one philosophy--expect more. Since 1995, we’ve been busy reinventing hospice by finding ways to spend more time with patients and families; and developing ideas to help them get the most from this delicate life experience. Crossroads is there when patients and their families need us most. Our patient-to-staff ratio is among the best in hospice. As a result, we provide more daily visits to our patients than anyone in hospice care. Our staff consists of doctors, nurses, aides, chaplains, social workers, bereavement coordinators, volunteer coordinators and volunteers, all working as a team to meet the needs of the hospice patient and their family.
