PMCN

Description

PMCN connects service members, veterans and their families to critical resources and services to meet needs that are complex by fostering coordination, collaboration and partnerships among service providers; training community-based providers; and conducting outreach to providers and our military and veteran communities to increase knowledge of and access to resources.

Through our network of partner agencies, web-based information and resources, and call-in assistance, PMCN supports the whole individual and family by linking them to advocacy, behavioral health, child welfare & family supports, crisis intervention, education, employment & workforce readiness, medical & physical health, social services, and transition & reintegration services. A key component of our services is to provide "triage" to connect people quickly to the right services (so individuals are less likely to give up and end up in crisis); and follow-up support through navigation assistance to help those with complex challenges who need longer-term, personal help.
