Telephone or In-Home monitoring of frail elderly Kaiser members' health and well-being.

We are a Geriatrics program serving hundreds of older Kaiser members at any given time. We help them to negotiate the healthcare and community-based system of care, and provide phone and in-person problem-solving and monitoring of their health and social well-being. Volunteers are key to help in providing frequent member contact in order to insure that members are stable and are managing okay in the community. Volunteers may also alert nursing and social work staff to any member concerns like missed MD appointments, signs of depression, or other needs for in-home assistance. Regular monitoring by volunteers allows clinical staff to stay on top of problems and prevent crises, and members LOVE the personal connection of the contact with volunteers.
