Description

Circle Family HealthCare Network (CFHCN) is a 501C (3) federally qualified health center that has served the Austin, Humboldt Park, East Garfield Park, West Garfield Park and North Lawndale community areas on the West Side of Chicago for 36 years. CFHCN is committed to improving the total wellness of the communities it serves through the integration of primary care, behavioral health services, and patient-centered support programs and services. CFHCN provides Behavioral Health Services, Children & Family Services, Early Intervention Program (EIP), and Community Primary Health Care Services through 3 clinics, 19 shelters, and a Mobile Health Team Unit (MHT). CFHCN’s MHT unit provides targeted medical, mental health, social and educational services on Chicago’s West Side to people who have found themselves homeless.
