Mission Statement
To provide the highest quality interdisciplinary care to all terminally ill patients, the bereaved, and their families. To create a climate where all staff can do their work. To educate professionals in the philosophy and art of Hospice care so that dying patients, their families, and hospice will have an adequate supply of knowledgeable professionals wherever they are. To build a community through education and information that can support patients and families as each person encounters those who need support. To provide donors the privilege to help others through their gifts to Hospice.
Description
Hospice provides most of the health and support services of a health care facility for terminally ill patients of all ages in the patient's home, nursing home, or hospital. Hospice services, however, do not end with the death of a patient. Following a death, Hospice bereavment staff continue to provide care to the surviving family members. Hospice takes a team approach to care for each patient and their families. Each team is made up of 2 nurses, social worker, chaplain, bereavment counselor, certified nursing assistant and volunteer.
