Health at Home is the home health agency for the Community Health Network, San Francisco Dept. of Public Health. We provide home health care for some of San Francisco's most needy. Of our almost 200 clients on service, approx. 25% have AIDS or HIV Disease, 25% are receiving end-of-life or palliative care services and the remaining 50% are individuals diagnosed with a life-threatening, chronic or disabling illness such as heart & respiratory disease, diabetes and fractures & other injuries related to a recent trauma. Most of our clients have recently been discharged from SF General Hospital. Many have issues around addiction, may be undocumented in this country, come from diverse cultural backgrounds and life-styles and have limited finanical and family/ community support.
