Mission Statement

Hospice is a program designed to support the recognition of death as the final stage of life. Hospice provides comfort care that focuses on the emotional, physical, and spiritual services to those patients and families with terminal illnesses. Guardian Home Care and Hospice allows for a transition for those home care patients that are in need of hospice services at a later time.

Description

The Hospice team consits of qualified personnel from a variety of disciplines including social workers, RNs, CNAs, chaplins and volunteers. As a team, we provide holistic comfort care and ensure the needs of the dying individual are being met. All members of this team are considered equally important and contribute to the well being of those we care for. Weekly meetings are held to ensure continuity of care and share ideas or concerns.
