Mission Statement

The mission of Heartland Hospice is to provide quality hospice care to patients with a limited life expectancy, as well as, support to caregivers.

The quality of life for those we serve is enhanced through palliative and specialized supportive care designed to meet the physical, psychosocial and spiritual needs of patients and caregivers.

Hospice care is provided by an interdisciplinary team utilizing an approach to the care delivered to patients either in their home or extended care facility: working cooperatively with area health care providers and other community resources.

Heartland Hospice strives to provied care to all who are eligible and need hospice care irrespective of ability to pay.

Description

Hospice is holistic apporach of care to help the terminally ill and their caregivers/family. This is done by providing not only nursing staff but also providing a Social Worker, Spiritual Care Coordinator/Chaplain, Bereavement Coordinator and Volunteers.

The Volutneers help fill the void created because family and friends stop visiting once a terminal illness has been diagnosed.
