The mission of PCC Community Wellness Center is to improve health outcomes for the medically underserved community through the provision of high quality, affordable, and accessible primary health care and support services. We are committed to serving the needs of men, women, and children in all lifecycles, while we continue to specialize in the delivery of comprehensive maternal and child health services to address this unmet need in our community.

Description

PCC Community Wellness Center is a not-for-profit community health center network with seven family health centers dedicated to serving you and your family in Chicago and the surrounding community. We provide comprehensive high-quality, accessible, and affordable primary health care, preventive education, and supportive services. At PCC Community Wellness Center we believe that everyone deserves access to quality medical care and our services are available to all without regard for ability to pay, race, ethnicity, gender, age, health status, religion, sexual orientation, or education. At PCC Community Wellness Center we take pride in being a resource to our patients and our community.
