The mission of PACE of the Southern Piedmont is to promote and sustain the independence of seniors wishing to remain in the community.
Philosophy:
The Program of All-inclusive Care for the Elderly (PACE) model is centered around the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible.
Description
PACE is an acronym for Program of All-Inclusive care for the Elderly and was created to provide seniors and their caregivers/family members the flexibility to meet the seniors entire healthcare needs to enable them to live in the community to end of life.
The PACE center in Charlotte is a large, welcoming space and has has a full medical clinic, physical/occupational therapy room and 3 adult day care rooms.
Seniors are provided care at the center during the day and are transported home in the evenings to be cared for in their own surroundings.
