The Family Choice Hospice team strives to uphold the dignity and ease the suffering to all that come under our care. We are dedicated to maintaining the comfort and care each individual receives as if it were given directly by ones own family. We provide a holistic approach to care and comfort while supporting the entire family structure. To achieve the these outcomes Family Choice Hospice works closely with family members and those individuals we service directly to plan and deliver all the services necessary to ensure the greatest possible experience and support.

Description

The Goals of Family Choice Hospice

To provide high quality palliative care, which enhances the lives of dying people and the people close to them;

To assure the long-term mental, spiritual, physical health and general well being of survivors after loss;

To serve as a resource to the community for education and development of high quality end-of- life care.
