Mission: PLAN of PA provides Planning, Trust and Care Management Services to individuals with special needs; autism, brain injury, mental illness and intellectual disabilities, utilizing a strength-based approach to maximize potential.

Description

PLAN of PA programs and services result in peace of mind for families of children with lifelong disabilities who wish to assure that quality of life and care continues when the family is no longer available to care for their loved one both now and in the future.

Created by Families For Families PLAN of PA offers three services to families of children and adults with lifelong disabilities: Care Management, Trust Administration and Representative Payee Services. Our staff works closely with families to develop an individualized care plan that is best suited to the needs of the family and the special needs client.
